Annual SHOT Symposium Application Form

  • Please note that there is a subsidised fee of £110.00 per person (non-refundable) to cover catering and hire charges.

  • The closing date for completed application forms is FRIDAY 30 JUNE 2017.

  • TITLE, NAME & JOB TITLE will appear on both your delegate badge and the delegate list. If you wish to opt out of having your details included on the delegate list you must advise us accordingly.

  • Symposium registration

  • Pre-Symposium meeting registration

  • Following the success of the Breakfast meeting last year, there will be a pre-symposium session from 08:30am - 09:30 am where you can 'Ask the Experts' who analyse SHOT data and write the report. Spaces are limited. Please see booking conditions below.

  • Please note: There is no extra charge to attend the pre-Symposium meeting, but places are very limited (you will be notified if you have been added to the wait list once maximum numbers are achieved). Once registered to attend the pre-Symposium meeting, if you are unable to attend, please contact the SHOT Office as soon as possible so we can offer your place to another delegate on our wait list. A charge of £20.00 will be invoiced if you cancel your place after Friday 30 June 2017 (this includes non-attendance on the day)
