Telephone request forms were drafted to provide a standardised template for telephone requests for blood components to ensure clear, accurate communication thus enhancing patient safety through consistent and traceable practices. The document contains 2 forms:
- Routine blood component request form
- Major haemorrhage activation request form
These templates have been provided in an editable format, and are available to be used by staff in any hospital transfusion laboratory to address identified gaps in communication during blood component requesting.
