SHOT Near Miss definition updated!

The SHOT Team have updated the Near Miss definition and reporting guidance following feedback from reporters.

The changes are intended to provide more clarity regarding which Near Miss and Near Miss – Wrong Blood in Tube (WBIT) events are reportable to SHOT.

These changes can be found on page 9 of the Definitions document.

Please discard any previous versions of definitions you may have printed or saved and update with this version revised February 2021.
